The PDF is read through the same structure engine the Markdown path uses, so what lands in Word is real paragraphs and real tables rather than one undifferentiated block of text per page. A contents section becomes a live Word TOC field that Word recalculates when the file opens, and a page number that the type-size pass mistook for a heading is written as ordinary prose so it never pollutes the navigation pane. Pictures are placed where they appear, scaled to fit the page. One limit is worth knowing before you start: a PDF carries layout rather than structure, so heading levels are a heuristic over font size and deserve a look before you trust the outline.

Do images come across?
Yes. Each picture is placed where it appears and scaled to fit the page. A very large document can reach the image limit, and the result then says how many images were left out.
What happens with a scanned PDF?
It converts, as pictures. Scanned pages have no text layer underneath, so each page is placed as an image and its text cannot be edited or searched. The result notes that no text layer was found.
Are the heading levels reliable?
They are inferred. A PDF records type size and position rather than document structure, so headings are detected from how large the text is. Glance at the navigation pane before relying on the outline.
Is there a page limit?
The first 500 pages are converted. A longer document still succeeds, and the result carries a warning naming how many pages the original held.
