Plain text carries no structure at all, so the PDF is the text flowed into pages with a default font and default margins. Nothing is lost, because there is nothing in the file beyond its characters and line breaks. Two things follow: long lines wrap rather than run off the page, and a heading is only a heading to a human reader — it arrives as an ordinary paragraph, because nothing in the source marked it as anything else.
